Please note 24-hours notice of cancellation or schedule change is required to avoid being charged for the  full cost of your reserved appointment.

In the case you cannot perform an on-line transaction or are having difficulties, please download our Product Order Form and fax to 212.366.1024 or scan and email to info@fabulalife.com with the completed forms and photocopies of the credit card front and back side to complete your order. A payment invoice will be emailed to you with the details of the transaction.
